Nice find.

However, I'm not sure if any doctors have actually been threatened with disciplinary action. I know several patients have been told by their doctors that they cannot prescribe more often because they could be dismissed, but they are just lying.

All doctors know that, if it is in the patients best interests and it is safe, then there's nothing to stop them prescribing 'off-label'.

I'm also not keen on the way they describe the serum B12 test measuring both active and inactive B12. That fact is taken into account by the different ranges. The range for serum B12 is five times higher than for the active B12 test. It's like counting sheep - count heads or count legs and divide by four, you get the same answer either way.
