Confused & would appreciate help..

Hi, a little bit of background, I have family history of P.A., as well as Hypothyroidism, I had T3 Toxicosis diagnosed in 2009, which was treated with Radio-iodine in 2013, so now am Hypothyroid too.

I wondered if I could ask for your help in determining whether or not these results either mean an absorption issue, a B12 Deficiency, and what the rest of the results mean please.

Some of my symptoms include, inability to eat properly, (due to pain in my abdomen, which is currently being looked into) Hair loss, Dry mouth, Insomnia, mood swings, lack of patience, poor concentration, brain fog, (many of these are hypothyroid related too) numbness and pins and needles type pain in my upper back, pins and needles in feet and hands, extremely painful joints, sometimes visual disturbances, and itching (similar to the feeling of something crawling all over you)

B12 289 ng/L (220 - 700.0)

Folate 2.1 ug/L (2.6 - 17.3)

Ferritin 34 ug/L (10.0 - 420.0)

Mean Cell Haemoglobin 32.4 pg (27.0 - 32.0)

M.C.V. 91fl (80 - 99.0)

I have other results but am not sure which are, or are not, relevant. Please let me know if there are any results I should add..

Hi Hidden "People with autoimmune disorders especially thyroid disorders such as Hashimoto’s thyroiditis and Grave’s disease" are at rick of developing a Vitamin B12 deficiency.

Your B12 is not particularly high whereas you are deficient in Folate which is essential to process the B!2 and your MCH is slightly elevated.

P.A. can be inherited so I suggest you ask your doctor to test you for Intrinsic Factor Antibodies and for Gastric Parietal cell Antibodies.

I am not a medically qualified person but I've had P.A. for 45 years and I'm still "clivealive" at 75.

Hi Hidden

Your B12 results are pretty low and are at a level that may cause symptoms of deficiency in some people. Are you getting treatment for your deficient folate levels and possibly iron too for your lowish ferritin levels? Have you had your Vit D levels tested?

I agree with clivealive . I think you should discuss these symptoms with your GP in relation to B12 - yes some of them can ovelap with those of hypothroidism but B12 could be the cause and I think they should be investigated, especially given your family history of PA. I would ask for an anti-intrinsic factor antibody test to test for pernicious anaemia. The test is prone to false negatives so testing negative does not necessarily mean you don't have PA. Other available tests are MMA and Homocysteine but these are not normally first line tests and can be affected by other things like your folate deficiency (homocysteine). It is best to not take supplements containing B12 until your GP has done their tests though the Anti-IF antibody can be done after if need be. (1 week or more after injection)

as taka and clivealive say your folate is certainly low enough to be causing the symptoms you mention - and you have signs of macrocytosis.

The ferritin may indicate that your iron is also low pointing to a possible absorption problem - so even if you can't get your GP to budge on the B12 I'd certainly ask them to look into what the absorption problem can be - PA would be one but other candidates would include coeliacs and possibly Crohn's
