I still struggle with hocd for my little sister, sometimes when i watch her i have the bad thoughts, sometimes i don't have them, but my mood changing in seconds, and i feel very tired, because this thoughts are literally eating me, I really don't know how to ignore them for long period of time

OCD attacks what is most important to you. It just shows that you care about her and in reality would never do her harm. I hate the way it clings to the things and people we love most and how rotten it makes you feel. I'm sorry you're going through this. You're not alone.
It's OCD, your brain making up all this, you don't have to listen to what it says to you. Like try not listen to it AT ALL. And if you do any compulsions related to this, please stop. If you don't do compulsions (including mental "pure O" ones), the obsessive thoughts within a few weeks will become less intense. The better you train yourself to stop compulsions, the less OCD you will experience. OCD is an ego-dystonic disorder which means in reality you truly care about your sister very much.
