Has anyone got a link to the letter for the 5th covid vaccination?

I can't seem to find one for me. I'm on ruxolitinib and have MF

just take along your personal medical details and copies of your regular medications.

you should be able to make your appointment online on NHS website . I took along my most recent letter confirming my condition but the local pharmacist could see on the system that I had had a 4th vaccine and the reason so didn't even ask for any confirmation of medical status.

I seem to be surrounded by covid cases at the moment so last evening I booked online for 5th jab this afternoon. I took my copy of the letter from mpn clinic to GP which covers my last clinic appointment and states my diagnosis and current meds. I offered to show it but didn’t need to as I was able to say (and answer a few short questions) why I was eligible for 5th jab.

All very straight forward and I hope it will be the same for you too.
