Penis swelling when erect.

I have left this for years as I am a typical male. I can retract my foreskin all the way back and forward when flaccid no problem. When I am erect I can just about retract the foreskin however it never goes back on its own.

The penis head gets very red and swollen and it starts to hurt. Only with a lot of manipulation and the penis starting to go soft can I pull the foreskin back over the head. When I masturbate I never pull the foreskin back fully for this reason but during intercourse where it does it can get painful and sore.

Should I see my doctor or just carry on living with it (42 now)? I have always had trouble peeing but never considered this may be the cause of struggling to get and keep a stream going until reading some other posts. I ask as if anything it's getting worse/tighter rather than staying the same. Thanks.

I suspect you do know that you need to talk to your doctor about this. Don’t put it off, you know the conversation will eventually take place. Make it sooner rather than later. You’ll feel better as soon as you make the appointment.

Bite the bullet and go and see the Doc,I put off going for years due to embarrassment and fear,when I finally went it wasn’t too awkward at all as the Doctor was so easy going and reassuring,as for the op itself it was done in 20 minutes with a local anesthetic and honestly I had no pain whatsoever.I promise you the reality is far,far less worrying and uncomfortable than the expectation that you’ve got going on in your head atm.

Doctor or sexual health clinic yes. They've literally seen it all before so don't be embarrased. You can request a male doctor and a chaperone if you're nervous.

You might be able to stretch it with steroid cream as you say it does go back with a bit of force. Don't try this without seeing a doctor though. Same goes for the phimosis rings sold online, get it checked out first.

My stream improved post circumcision but in my case I think it's because the balanitis (which affected the meatus) has cleared up and the glans is just less swollen. Still don't have great directional control but peeing is much better.

There are plenty of options short of circumcision so don't commit to that. It might be that a partial circumcision or a dorsal slit (en.wikipedia.org/wiki/Dorsa... or preputioplasty (en.wikipedia.org/wiki/Prepu... will work for you. Keep your foreskin if you can.
