Endocarditis complications

My dad was admitted to the hospital 23 days ago with what was determined to be infectious endocarditis. He was moved to the ICU just over 24hrs after being admitted because he had two seizures. The seizures were stopped and ct and mri scans showed several small strokes and a larger brain bleed in the right temporal lobe. His breathing was compromised due to the stress of all the events and the infection so he was intubated. Bleeding and seizures were stopped and infection improving within 24hrs. He was sedated to help with discomfort and heart rate. They tried twice to remove intubation within the first two weeks with little improvement but not enough to be fully in his own. Fluid was building up so CRRT dialysis was ordered. He tolerated it well and was switched to as needed dialysis. During his time of sedation he was still responsive and would follow commands, squeeze out hands, and blink in response. After dialysis port was put it they increased sedation and pain meds and he was completely knocked out for about a week. Tracheostomy was done last week. Since then, he has been off sedation and pain meds. All numbers are improving, breathing at cpap setting even, responds to pain stimuli, opens eyes, but will not follow commands. Doctors are not helpful/hopeful and blaming it on “everything he’s been through”. They said he’s not a candidate for dialysis (even though he’s tolerating it well) and are pushing a “comfort care” route. Is it possible he’s still just working sedation and general anesthesia out of his system? No new ct or mri’s have been done. They have also placed him back on blood thinners bc of his leaky valves (replaced both valves once already two years ago). My family is still hoping for a miracle. Any similar situations out there? The doctors are very discouraging and not offering anything hopeful.

Especially with him having been on dialysis & having limited renal function - this will slow down his body’s ability to expel effects of sedatives which will slow down how quickly he wakes

I had my artificial aortic valve replaced early and finished up ventilated / sedated for 6-8 weeks. They tried a number of times to take me off but I wasn’t ready and I came round in my own time. Came out mid March and well into recovery. (Ps 72 yes old)
