Moving house?

Hi, has anyone experience of moving house and therefore having to transfer cardiology hospital monitoring support, I have an ICD fitted, heart medications etc? Who did you ask to effect the transfer, your existing Cardiologist, your GP or your new hospital area? Any answers much appreciated.Cheers #chinkoflight

When I moved my electrophysiologist transferred me over to a colleague he worked with a time ago in the part of the country I was moving to. Brighton. It was pretty straight forward.

That's really helpful. I wasn't sure whether I had to make the change through the GP or the hospital. Can't find any guidance. Thanks pip.
