Most interesting recent posts and replies. having had trab & three needlings right eye within the past four years, trab left eye 30 years ago and revision of that two years ago I am now due revisions of both shortly. I scar up despite mitomycin and drops aren't doing the job of keeping IOPs < 10 as desired. One question:- my instructions tell me to use my drops on the day of the procedure. I would have thought it would be more useful to judge the degree of surgery knowing the "natural" IOP level. What is the experience of others please?

Hi I've just had the one Trab plus a needling procedure and several laser procedures and my memory is that I was given the same advice for all - i.e. to take the drops in the morning before the procedure. I think you raise an interesting point and I guess it depends how you look at this. Either you have results that compare the previous treatment (eye drops) with new treatment (Trabeculectomy) which is interesting (how much better is the Trab at managing pressure than the drops) or as you suggest, assess the overall impact of the surgery by comparing with unmanaged pressures.
I'm not sure how long you need to miss eye drops before pressures are completely unmanaged (e.g. if you don't take them on the morning of the procedure would that result in the pressure reading you are interested in?).
It would certainly be an interesting discussion to have with the surgeon when you have the next procedures.
