I do not know your area very well, only been to Birmingham half a dozen times. Do you know where your local Job Centre Plus is? Give them a ring and they will be able to tell you, it will most likely be them anyway?

Hello we are in Oldbury and the address I have may be different to yours depending on where you live In Birmingham,

As Ken says your best bet is to ask your Job centre. I do have an address but there is a severe warning not to send them to the wrong place, If you do it may then take weeks to sort out.. Good luck

Get in touch with your JSA and they will tell you or you could book and get them to send by fax at their office. does not get lost then. I have found that sick notes over 3 months are often rejected.
