Yes you need to fill it in DLA and ESA 2 different benefits xx

You need to call the DWP to find out the purpose of the request - I.e. is it for DLA or ESA. I have the same DLA award as you but I have end for ESA and a medical to attend...

Yes! Regardless if you have been awarded DLA or not. You need to fill in the ESA50. Try to give as much information as possible on the ESA50 and also give as much supporting evidence as you can.
