Anyway today I'm here to ask something else for a friend of mine.
She is with an extremely low AMH and can't find a decent doctor herself and is doing some research.
For you ladies with a low AMH what protocols were you on? Were you on low, or higher doses or stimulations?

Hello! My AMH was considered low when I started my treatment last year but I also have the advanced age factor (43 at the time of EC) so my protocol may not suit your friend. My clinic did not just want to go as per AMH but also looked at my antral follicle count (13) and FSH and E2 levels (both slightly raised indicating diminished ovarian function) during the Ovarian Reserve Test which I seem to recall they did on Day 3 of my cycle. Your friend's history will also be relevant, has she achieved natural pregnancy, is she ovulating regularly? My stims protocol details and the name of my clinic are in my profile if you or your friend would like to take a closer look. I apparently responded better than my consultant expected and over the course of two back to back embryo banking cycles we achieved 5 good quality blastocysts which we had PGT-A tested, now we have 2 euploid embryos that we're hoping to transfer soon. I was also on DHEA and CoQ10 and Alpha Lipoic Acid for three months prior to starting my egg collection cycles, I am convinced this has helped to increase my egg quality. Melatonin during the stims phase may also have had a positive impact and I also injected growth hormone on alternate days during stimulation. Delighted to see your treatment is moving along nicely again! Best of look on Monday. So, I'm not sure what level is technically considered low but on my first check of it back in 2018 (I was 38), it was 8.4. Successful cycle. Retest in 2020 was 5.7, took a few goes, but eventually successful. Then in November 2022, 6 months after birth, checked again and it had risen to 17. They found it unusual and checked 2 more times but it was still 17. Said it happens. However, my protocol didn't change one bit from when it was lower. 450 gonal F and 225 Luveris. I'm starting stims for next round tomorrow night. My consultant has said that my stims is the highest dose she can give but I'm not sure as a friend told me she was on 900 gonal F and 225 Luveris but lack of ovulation was her issue. We do have one in the freezer but when we heard the AMH had risen, we couldn't not try fresh again. This is cycle 2 this year. Best of luck to your friend. I have low AMH but haven't had it checked now for 2 years so I'm sure it's dropped more.
I was initially put on long protocol and this resulted in a really poor outcome, no eggs collected. The clinic subsequently said maybe the down regulation had shut my ovaries down to the point that they struggled to wake back up again even with the high stims.
Since then all my cycles have been short protocol (no down reg). This has worked much better for me and the no eggs has never been repeated. Obviously I am still waiting for success so I can't say that it has worked but in terms of better egg collections and blastocyst results, based on my experience I would suggest short protocol for low ovarian reserve.
I have always been on high dose 450 and have tried different stim drugs as I've had several cycles. I've heard low stims can be good for quality but I've never yet tried that so can't really comment xx

Hello. I know lots of women with low AMH do manage to conceive successfully through IVF treatment, so fingers crossed things will work out for your friend. However, extremely low AMH can mean very low chances of success. Mine was extremely low (0.4) and we were given <5% chance. Your friend should ask any clinic she approaches for a percentage chance before making any decisions.
We decided to give IVF with my eggs one shot. It was standard short protocal with the max dose of stims (450). Sadly, only one follicle developed. They managed to retrieve an egg but it had poor morphology and didn't fertilise (there's no direct measure of egg quality, but tends to be a correlation with low AMH/low quantity). The consultant confirmed it wasn't worth trying again with my eggs.
So we moved on to donor eggs and that has gone much better so far (viability scan this afternoon so we'll see!). I really hope things work out for your friend with her own eggs, but if her AMH is really low, she may need to consider donor eggs at some point - might be worth starting to get her head around the idea now xx
Hi 👋 great to hear from you xx I had a low AMH my clinic also looked at FSH they did have a maximum FSH for treatment so would be interesting to explore that (although can be massively variable, mine when from 18 to 12 for example ) and the follicle count too this gives a better picture to base potential treatments xx
I did two cycles on short protocol highest dose and then one cycle on long protocol - the latter worked better for me and resulted in successful twin pregnancy. We would have moved onto donor eggs had that been unsuccessful too x
Hey lovely, I am zero help on the protocol but just to say heaps of luck for Monday, I kind of don't want them to find something but also would be fab if they found something tiny and immediately fixable that didn't delay anything and made all the difference for your transfer in May if you see what I mean... that magic fix! For an extremely low AMH (mine 0.16) no amount of high or low stims worked. Not even clomid could create 2 eggs per cycle. Just kept ttc with that 1 egg per month till I hit a good one.
