Hi girls. I read some information about ERA test but am interested to know whether it is necessary to do ERA test if there was implementation but end up with miscarriage before? Will our body change as time goes or our implementation window will never change? I also read to adjust progesterone based on the results of ERA. Any comments on the subject is welcome. Thanks a lot.

Hi lovely, I have had the ERA test done. I did it because we'd had 3 negative transfers and therefore classed as recurrent implantation failure. I'd never been pregnant nor had a miscarriage. The idea is that they check your window of implantation to make sure they are transferring the embryos at the right time. If you're miscarrying then it's possible that it's not a timing issue - as your embryos are implanting, just not carrying on growing.The results of my ERA showed that I need an extra 24 hours of progesterone before my uterus is ready for transfer. xx

Hello! I am looking at this now. Does this mean if you have a 5 day blastocyst you have to transfer it on day 6 if that’s your implantation window? Is it the same window every month...

I'm not sure about the age of the blasto - I only have day 5 frosties so it didn't occur to me that it might be different with a 3 day-er.... you'd have to check this with your clinic, they would know for sure.So you take meds to prep for a normal FET - then on day 5 of progesterone (when you would normally transfer) they take a biopsy. The assessment of the biopsy tells you when your uterus is at it's most receptive. It could be before they took the biopsy or after (or it could be exactly when they took it in which case nothing would change).

For me, I was pre-receptive meaning that my window of implantation was AFTER they took the biopsy (by 24 hours). So my transfer this time will be day 6, rather than day 5 which is standard. xx

I am doing the ERA test this month. It is medicated so which means whatever the result will be, the FET will be using the exactly same medication/duration in order to have the exactly same implementation window. Even there is any adjustment needed, it will be based on the adjustment of medication to match the ERA results. I had nature cycle transfers with day 3 embryos 3 times before with implementations (although ended up with MC) but couldn’t implement for the recent day 5 blastocysts transfer twice with 4 blastocysts so I guess there is an off for 1-2 days and previous day 3 transfer could catch up the end of the window but day 5 missed it? Even my previous clinic said my recent cycles are too long so probably nature cycle is not good enough for me. I was doing ivf in France which has a lot of restrictions as not allowed to do PGS and always nature cycle transfers if I can ovulate. My understanding is even the implementation window doesn’t change but need to have the same medication.

I had 2 early losses before we had our ERA which showed I needed longer on progesterone so it can be the case that you can have implantation. The ERA basically tells you how long you need to be on progesterone to optimise your chances for catching when your endometrium is most receptive.xx

Thanks. May I know how many weeks for your previous 2 early losses? When I had my miscarriages around 5-6 weeks after transfer, I was on nature FET so they didn’t give me any extra progesterone and didn’t even bother to check my level of progesterone........ is that possible I didn’t have enough progesterone for the embryos to grow?

I had a spontaneous early loss at around 6wks and the other was around 5wks and probably classed more as a chemical pregnancy to be honest as my hCG levels started to drop on that occasion and we got advised to stop meds. I did have issues with my progesterone levels where I wasn't absorbing the meds and had to be on a higher dose for future transfers (checked my levels on transfer day or the day before) but the amount of progesterone isn't related to the ERA which is more the time you are on progesterone. if that makes sense.xx

I have had 1 early loss and 3 recurrent implantation failures. I was advised by my clinic that these are '2 sides of the same coin'. Decided to go ahead and have the ERA. Maybe my embryos just arent strong enough to implant and stay (never had them tested) or maybe all my transfers were just missing my optimum window, and the 1 mc I had just caught on but wasnt quite the correct timing to thrive, if that makes sense. Who knows. However my ERA did show I need 12 hours extra progesterone, trying this out with my transfer I am having next week 🤞

Thanks, it makes sense now. I am going for my ERA test next month. True, it could just be out within half day so the MC embryos could catch the end or begin of the implementation window but not the perfect environment. And after 2 miscarriages I had 3 recurrent implementation failures so my new clinic advised me for ERA test. Anyway, who knows, it is always good to rule out one more issue. Lots of good luck for your transfer.
