Hello, I was diagnosed with advanced fibrosis f3/4 and fatty liver. My fibroscan in 2022 was cap 297 kpa 11. Six months later it was cap 336 and kpa 12.2. I just had another fibroscan and the cap was 278 and kpa was 12.8. I am happy the fatty liver has improved, but the scarring is worse. I also show Granulomatous hepatitis of unknown origin on my biopsy. I am trying to understand why fatty liver is improving, and scarring is not? I still have 75 lbs to lose. Have lost 30.

I think 11, 12.2, 12.8 kpa are pretty the same. Fibroscan is not so accurate. I think the most accurate about fibrosis staging is biopsy.

Hi Sam

Those readings are within the margin of error, but this is a slow motion disease. Losing the weight is the first step and assuming your diet is anti-inflammatory that takes the pressure off and gives the liver a chance. It took a decade or two to get your liver to that point and it takes time to see improvements in fibrosis. Some people never succeed at reducing the scarring but if this is typical NASH you should see some improvement within a couple of years. We fight the battle every day but the war is a lifetime thing once you get to advanced fibrosis.

Thanks for your reply. My biopsy also showed granulomatous hepatitis so I don't know if that is also at play. So do you think if my fatty liver is improving and scarring is not that it might still improve? I am just concerned because the fatty liver was better and scarring was not. I still have weight to lose so hopefully it will help. I also hope it can still reverse. So close to cirrhosis numbers. Thanks

Hi Liz

The most important thing is to stop the progression. If you have enough working liver to function and you can stay where you are that is a win. Better if you can make some progress over time on the fibrosis, but that is scar tissue which changes very slowly. You might consider the information on our site. It is what I did to get mine under control.
