Hello. A while back made a post...

Here is an update, so to speak.

Was referred to local gastroenterologist...

Alot of talk...was concerned with the high

Fibroscore and fib 4...said the key was

Weight loss, no difference in treatment

From nafld to fibrosis...said he had sent

Pts to be enrolled in trials, never accepted,

Wanted to do fibrotest, even tho dx with

Gilbert's, and said liver biopsy not

Needed.

We decided to go to the university liver center. First, they had not used fibrotest

For some time, and it was counter indicated

If you have Gilbert's !!

Said no to Fibroscan. Said would have

Difficulty since I'm overweight, plus would

Not tell us the most important thing, if

Cirrorsis, and to what level.

Recommend liver biopsy, would then

Know what stage, which would be needed

For research study enrollment, and also

If we needed to start to monitor for cancer.

The cirrorsis tissue can try to start to

Regenerate, and those cells can be

Cancer.

Not happy about biopsy, cost

And possible complications

What?? Regenerating cirrhosis cells can be cancerous? Am I missing something? Never heard of it before!

Anyone with advanced fibrosis does need to be monitored for liver cancer. The disruption of the liver cells associated with the inflammatory processes can lead to that cancer. For clinical trials the FDA still expects a biopsy so if you are considering a trial that would be a reasonable strategy. It is true that if you are too overweight the FibroScan can't get a reading but it is an individual thing depending on just how the fat is deposited over the ribs.

Well here is an update. Due to COVID 19 things have been put on hold for a while.I have had a ultrasound, and showed enlarged spleen, and slightly enlarged liver,

the billirubin is running about 1.4 and platelets are running in the 90's.

The liver center (250 miles away), has been wanting to do a biospy, at first a

fibroscan, but now wanting this, but difficult getting getting it all done.

They tell me now, if I don't do the biopsy, they will consider me with chirrious,

and require an ultra sound every 6 months, along with checking for varricies,

and also do labs with markers, and visit the center every 6 months, and be monitored

for cancer and later a transplant.

but, if a biopsy, i could maybe get into a study, but, then again would require to do the

above !

just getting tired.
