Fibroscan after one year from last one

I would like to share some upate about my liver status. A year ago I had fibroscan 8.8 kpa (IRQ/median 32% so not so reliable). No CAP score was measured. At that time BMI around 25.

Now today fibroscan 7.0 (IRQ/median 4%), CAP 237. BMI around 23, no alcohol during last year (but I have never drunk unhealthy amount but still).

My hepatologist said F1/F2 and S1 (here 237 not sure but I think is some border score between S0/S1).

Anyway I think my CAP score was higher a year ago despite the fact I was pretty slim that time. But I am happy it is going to right direction. Now my INR (the only test regarding liver which was abnormal) is 1.06.

Here my story

"I have Gilbert's Syndrome. Sofar findings: heterogeneous echotexture on ultrasound, fibroscan 8,8 Kpa (but with IRQ/median 32% rather poor reliable as I found), INR 1,28 (but after 2 months it decreased to 1,13), Ldl slightly high 140, total cholesterol 215, witamine D first 19,1 ng/ml after 2 months increasing sun exposition and supplementation it went to 36,9 ng/ml, bilirubin around 2,8-3,5 (but Gilbert's Syndrome explains this).

BMI around 25 when first tests, now around 22-23, no alcohol level to cause any damage. Ruled out (at least based on current tests) HCV, HBV, AIH (no antybodies like ANA, AMA etc.), PSC, Wilson, hemochromatosis. All other tests like AFP, platelets, albumin and so on in normal level. No symptoms at all."

Way to go. You are certainly on a right direction. Keep doing what you are doing and it looks like its certainly working out for you.

Just FYI, Fibroscan cannot be very reliable in giving accurate results for fibrosis and liver biopsy is considered the only gold standard in measuring the liver fibrosis accurately out there for now.

If you do not have any symptoms (or) your symptoms are not worsening then certainly you are on right track. if your hepatologist has determined that NASH is the root cause and ruled out all other possibilities then it certainly looks like lean NASH. BMI cannot be the measure of the amount of fat accumulated in your liver. Working towards healthy lifestyle changes is always a good step towards fighting lean NASH. As they say recovery in case of NASH is a marathon and not a sprint.

Thank you for information. Not sure if I have NASH or only simple fatty liver. Only biopsy would tell this but with my current results are too risky to try it (I asked about it doctors when first fibroscan both said it was not a good idea). I know fibroscan is only an estimation especially within relatively low values.

Yes, your results are not too high to get concerned and go for biopsy. Biopsy comes with its own set of complications.

Whether its a NASH (or) just Stage-1 NAFLD, healthy lifestyle with good diet & workout regime is the only way forward. Having a healthy lifestyle is good in terms of your liver health & also overall health and outlook.

Certainly you are on right track as things are working for you. Keep doing the things you are doing. Good Luck.

All normal (besides INR and vitamin D as I wrote but now both well in range). For example:

1. 8 years ago -> Fib-4 = 0.7

2. 1 year ago - > Fib-4 = 0.7

3. Now - > Fib-4 = 0.56

about fibroscans, what happens when that pulse or device signal hits a bone do the numbers go way up? And should the technician realize this ?

That's why you have median of minimum 10 values. One value out of range of others is irrelevant. Also to have more valid result IRQ/median should be less than 30%.

Of course technician should know where your liver is and measures it not something else.

Those Fib 4 are all good well below 1.30
