I have been diagnosed with endo cysts and a swollen fallopian tube via CT and ultrasound.
they said in surgery pending how bad they may have to remove one of each (they are unsure until they do the lap) but my concern is that if they are bad, could I wake up to a full hysterectomy? And if so why?

It’s easy to panic after a new diagnosis, specially if your doctors haven’t explained things very well. (I’m definitely a natural worrier!)
Please remember that it’s your body and you get to make the decisions about treatment/surgery. They are supposed to ensure that you have all the information you need to help you weigh up the pros and cons, risks and benefits before they do anything. If you’re not sure about anything you have the right to ask questions. You have the right to change your mind and withdraw consent at Any time during the whole process.
They also have an obligation to mention the worst case scenario, which of course sets off our anxiety, but if they didn’t tell us all the risks (no matter how tiny) and then something happened, they would be opening themselves up to a law suit.
It sounds like you have consented to surgery for removal of cysts and possibly a tube. You can explicitly refuse anything more being done (except in the unlikely event that there was some sort of emergency). Strictly speaking, if they find more than was picked up on your scans, they should end the surgery and come back to you with a revised plan.
