2 surgeries for 1 cyst - is that normal?

Hey guys, I saw my doctor today and she told me that the best option for my 6.5cm endometrioma (affectionately called Spud amongst my friends) is to have 2 surgeries, one to drain it and then another a few weeks later to remove it fully. It looks like this is going to happen over the next 3 months.

Has anyone else had a similar procedure? How did you manage to plan your work/ life around it all?

Would love some wisdom x

That’s strange- my cyst is 9cm and it will be getting done all in one go. Why do 2 surgeries????

I had a 5cm chocolate cyst removed 2 yrs ago but this was done during a lap to laser endo lesions and sort the ovary out. Have had another lap since that one July last year to do excision on adhesions. It sounds odd to drain and then remove...being put to sleep twice is not ideal and work......

I am thinking i may have a cyst again which I have been told is quite likely (unfortunately) as my pain is back to that original pain.

As long as they get it sorted for you trust they are doing it in the right way for you.

i have seen a few people on here have said they had surgery done in two stages. is your surgeon a bsge specialist? i suppose the question is why do they feel it cant be done in the one surgery? i had a 5cm endometrioma which was removed in the one operation. It is common that there would be other endo lesions when you have an endometrioma so it maybe due to how long they would expect surgery to take. my surgery took around 4 hours and i had extensive stage 4 endo excised too.

I had an 9 cm x 6 cm endometrioma excised in one go (2 hour procedure), and then a second surgery later to un-stick my ovaries, remove the rest of the endo and adhesions, etc. This was by a BSGE specialist. It was in two stages because the ovary was enlarged and time was needed for the swelling to be reduced to be able to work around it, and also to give more time. But the cyst itself simply excised, not drained first, I believe.

Hi,

Just to say this is exactly what happened to me almost two years ago. I saw a BSGE specialist and he wanted to do it in two stages too. I have stage 4 recto Endo. At time I was in a lot of pain I had two chocolate cysts both around 9cm I was also put on GNRH to put my body in temporary menopause the reasoning behind this was I was told to try and stop them growing or srink the cysts (they didn’t reduce in size) although my pain had reduced a little. As I needed surgery the specialist wanted to do it in two stages as the cysts were big they felt it would be better to drain them first rather than try surgery and work around them as they felt this would be difficult. I’m not sure if this is normal practice.

I wasn’t put to sleep if I’m honest I would have rather have been I found the drainage painful and had to raise my hand a few times to ask them to stop. They did give me something to help with the pain during the drainage but I could feel the tugging - although I had two and they were quite big and difficult to drain so this might be why I found it painful. It might be different for you, that was my experience. Not one I’d want to repeat I’d rather have the surgery and have it done at the same time but that wasn’t an option for me. Almost two years on and both cysts have returned although they are smaller currently but still causing problems I’m waiting to see a specialist again. I would ask your consultant what the reason is for draining first and any questions you may have so you know what to expect. I hope that helps a little.

No, that's not how endometriomas are removed. A friend of mine had a 10-cm chocolate cyst and got it removed right away along with some extra endo with one laparoscopy (performed by experienced excision surgeons). She was out of the hospital and went back home with both of her ovaries within 4-5 days and now is back to normal :)

Hi there.

It's not something I've much experience with honestly, but I very much agree with dev1984, as long as you have faith in your consultant, and they're and specialist and all the rest, then I would say there must be good reason for the two stages, I don't think they would put your body under any uneccessary stress if it weren't for good reason. Like some of the other ladies have said, I suppose if they remove it or reduce it in size some how and then go back a few weeks later it might make them more confident that they haven't missed any adhesions or endo because of the size and position of it. It sounds to me that it must be the most successful way or getting rid of your cyst and being sure they attend to any other internal issues to the best of their ability.
