dealing with wounds from surgery without a flat stomach?

I have always wondered this. Anyone who, like me, does not have a flat stomach how did this affect the incisions that were made? I have some fat and also a bit of a stomach apron due to past weight loss, this comes down over my bikini line. I see pictures of women with incisions in various spots but it concerns me that I would have a horrible time healing because they would need to make incisions over the ovaries/uterus which is where I have excess skin/fat. Anyone have experienced this?

May just need to watch the cleaning of it afterwards.

I was left with a nasty overhang after my first op, pretty much got a tummy tuck with my hysterectomy though 😁
