My thoughts on this are:

1. For my mum (CKD3/14y), we have adopted a Nutritional Therapy approach which is holistic and hence addresses all systems including immune.

2. The paper is largely focused on drugs for treatment which sadly is common. For research papers like this, it is generally for commercial reasons as the investigators seek to get funding for developments from pharma.

3. Re natural, there is an acceptance that "these treatments hold promise, their effectiveness and safety may vary in different patient populations and stages of kidney disease".

Whilst I agree with the points about populations and stages, the point on safety appears to be a comment unsubstantiated by any scientific evidence. As we know, its the safety of drugs that is the key issue, yet that isn't mentioned (no doubt for reasons oulined in 2 above).
