I am diabetic and I found that, my BP is highly fluctuating from 115/70 to sometimes 147/ 93. I am using digital instrument for my BP measurements. I want to know from this noble forum that how reference BP should be taken for knowing the actual BP and at what BP level, medication is advised. My HbA1C remains between 6 to 7. and normally below 6.6%.

Seems you are fine. You have not mentioned your age and what time of the day you have taken BP reading. Also readings in digital instrument fluctuates. Take average of three readings. If you are young try to maintain 130/80 with diet, exercise and controlling stress levels. If it consistently shows high follow medical advise.
Any BP reading higher than 120/80 , after consulting with your physician you must take a BP medicines such as ARB or ACE...
In fact it is a standard practice nowadays for any Endocrinologist to prescribe one of the above two Medicines if you are a diabete. Even if your BP is ok, a low dose Is a standard protocol.
Apart from BP lowering, the function of the ARB/ ACE for a diabete is to protect your kidney from Nephropathy and also help in endothelial health/ Nitric oxide production.
BP lowering medications such as: ACE/ ARB nowadays are considered safe on long term without any side effects and the benefits outweigh the risk you would carry by not taking it!
How many of you understand "ARB or ACE"?

ACE is the enzyme in blood converting a precursor to blood pressure raising substance normally.its inhibitor is used to reduce BP in patients

ACE inhibitors ( Angiotensin converting enzymes) and ARB ( Angiotensin Receptor Blockers ) are a class of life saving medications used for hypertension / high blood pressure and are in the market for the last 30 years...
Apart from blood pressure lowering , these drugs are also found to protect kidney if one is vulnerable to Nephropathy due to metabolic Syndrome/ type 2 . These are also found to Protect endothelial health.
For diabetes even without High BP, it is nowadays a standard protocol to prescribe a low dose Telmisartan (ARB).
So much so, even without diabetes and high BP, in case a person is having High creatinine and albuminurea, a nephrologist would prescribe ARB/ ACE.
Fortunately these drugs are highly safe if taken for long term and life long with out side effects. Some people may have a little sore throat with ACE , but then that is how ARB came as a new discovery to have zero side effect. ...

measurement of BP to be done morning after getting up in a lying down posture at the elbow level.you can repeat in a upright position also if you have significant postural change. this s the base line level.
measurements at other times are unreliable.
