My suggestion take PPBS after each meal, i think after certain meal the sugar lvelvs increasing, dont just stick to after breakfast
Recheck the values , otherwise you are in danger . You certainly need insulin.Talk to Diabetologist immediately .

There is no wonder in having such a variability in ur glucose values and A1c value. This may be due to the glycemic variability you encountered. That means your glucose levels differ according to the difference in your meals and physical activity pattern in different times of a day. If you could make a record of your blood glucose in different times you may spot out at what time of a day the blood glucose increases if at all it does.
Record your blood glucose minimum of 5 times a day using a glucometer at the following timings.
Before breakfast
Two hours after breakfast
Before lunch
Two hours after lunch
Before supper.

Switch over to LCHF. Check HbA1c after 3 months.
Don't worry there is a solution. It requires a strong will and discipline. Low carb is the real answer to the diabetes problem. The food is good and you can easily stick to it.

You have to go to a doctor. I live in Mumbai and went to Dr. Nadeem Rais a specialist. The patch has to be put out side the arm and it stays there for 14 days and is controlled by a handheld computer.

FBS & PPBS are mainly the reflections of activities of that & previous days where as HbA1c reflects the activities of previous 90 to 120 days. Hence HbA1c need not be directly correlatable to FBS & PPBS.
At initial stages of high blood sugar detected, generally common. The patient has to follow remedial measures strictly, preferably diet, morning walk, yoga, medication etc. It can be controlled and bring to normal level within 2-3 months, provided you strictly follow the remedial measures. I experienced the same problem and felt depression. On consultation and supervision of doctor, I could control and brought to normal level within 1 - 1 1/2 months. (FP- 350, PP >450) TO 145-175.
If FBS, PPBS (breakfast), PPBS (lunch) & PPBS (supper) are maintained at the reported FBS-PPBS values for 3-4 months, HbA1c is bound to come down drastically.
