What is your daily meal plan?
Morning-one cup tea with 1/2 t.s. sugar and 2-3 khaki as.
Lunch 3 roti + one bol dal + one bowl subjI + 2 st rice.
At 4 pm tea and nashta

Looking at your intake it is mostly carb loaded. You are hardly eating protein and fat. First thing you have to do is gradually reduce your carb consumption. Stop adding sugar to anything you drink or eat. Then stop eating bakery products and all processed food. Reduce your chapati/roti. Avoid rice totally.
Replace roti with sabjis and salads. Add sprouted and cooked beans to your meals. Peanuts, almonds can be taken for in between snacks.
Whatever carbs are reduced have to be replaced with protein and fat.
Slowly add butter, paneer, cheese to your diet.

Even 1/2 teaspoon sugar is very bad. Go sugarless for tea and request you family to totally, I mean totally avoid any trace of sugar, jaggery, and other sweet dyr fruits like kajur, mangoes, bananas, chikoos, grapes etc. Consultant a good endocrinologist and stick strictly to that medicine, diet, exercises.
