Unusual spike in blood sugar.

I have nursed Diabetes in the last 14 years and kept it under control. With change to my lifestyle and diet as well as Metformin and Glanil.However, in the last 20 weeks , I have experienced a troubling spike despite my self discipline. Going as high as 240! I got on Zoloft, antidepressant, and brought it down to around 140. But has refused to go down further. Has anyone experienced this and what can I do?
