Hi everybody, my dad is 86, about 13 years of PD history. He’s more and more pained by non-motor symptoms, speech, and cognitive impairment mainly. Has anyone experienced the use of thiamine in similar situations? Thank a lot

Unfortunately, most of the members who are either testing or using HDT/B-1/Thiamine are not at the age or more advanced stage as you are describing your dad or at least they have not said as much.
On the positive side, Dr. Costantini who is the Italian neurologist who invented the treatment has previously stated that all patients who respond to HDT have shown improvement and the minimum improvement for responders that he saw was a halt or very significant slowing of disease progression for 7 years and counting in his patients who are at more advanced stages of the disease, but he also said that some of these more advanced patients also saw multiple symptom improvements.
On this forum, one member who was giving his wheel chair bound father, who was at stage 5 of disease progression, HDT (High Dose Thiamine) and that allowed his father to walk again and is now at stage 3.

Thank you very much charist, I’ve seen the page, but I’m afraid to start with a very high dosage in such a fragile person. May have a sense to start with just 500mg it less, or even see if a common B complex produce any effect? How to contact Dr. C. collaborators? I’ve written an email to the mail address reported, not sure if they read it. Thanks for any advice

Something to keep in mind when it comes to HDT dosing, there is no one size fits all and to clarify this point further for you, there are at least three members who are doing well on "less than 100 mg of B-1 per day total" and there are at least three members whose effective dose is 4,000 mg per day. So although the therapy is refereed to as High Dose Thiamine, it is clear that some people simply require less than others and that is often based on weight, time since diagnosis and severity of symptoms.
