That seems like a difficult question to answer because B1 is produced in a healthy gut, so that would suggest it is always available for use by the body, but PwP have a perturbed gut microbiome which may hinder or halt natural production of B1. The liver is thought to store B1 for up to 18 days.

More specifically, if I take 2 doses of vitamin B1 during the day, what is the duration of effectiveness of the vitamin in number of hours?

Two doses a day, so I will assume you are talking about HDT. I can only tell you what I have seen reported on this forum, by those who have reported it worked to reduce their PD symptoms to varying degrees. The time to effectiveness has varied from one day to 90 days or in a few cases, even longer and the average seems to be at least 30 to 40 days days to see improvement. With daily oral use, the benefit seems to be maintained, but the time required to find the optimal dose has varied significantly.

In a human study where B1 was tested at 200 mg /day, it was shown to very significantly lower the inflammatory mediator IL-17 by fourfold, which is higher up in the inflammatory cascade than Il-1b, IL-6, IL-8 and TNF-alpha and in studies has been shown to be significantly elevated in PwP and other neurodegenerative diseases. IL-17 is usually at or near undetectable in healthy controls.

It is a good idea to note that B1 is also active in other areas than in the plasma. B1 has a significant effect on the gut microbiome.

I am very glad to hear it and congratulations!

Something that you may or may not be familiar with is a phenomenon that "some forum members" have reported after being stable on B1 for 6 to 18 months, a sudden worsening of symptoms which at first they thought was because B1 was failing, but Dr. Costantini described this effect while he was still here and it apparently is due to a reduced demand for B1 by the body, so they are essentially experiencing B1 overdose symptoms. I wrote about it here :

Being aware of this potential will go a long way toward removing any fear of B1 failing if it suddenly appears for yourself and is easily remedied. Keep in mind that not everyone reports having this issue with B1, but it is good to be aware of the potential for it. Continued success to you!
