1 colonoscopy + 1 sigmoidoscopy in 3 years; symptoms persist

In the past three years I’ve had a colonoscopy (July 2020) that was clear and a sigmoidoscopy a year later in May 2021 that found small internal hemorrhoids and a hypertrophic anal papillae (benign). However, my symptoms of rectal bleeding, some rectal pain, and intermittent constipation have persisted. I will say I’m not as constipated as I used to be so that has improved, But the rectal bleeding and dull rectal pain persist.

Frustratingly, I recently saw a new gastroenterologist. In August of this year (Aug2023) I had a CT scan of the abdomen and pelvis, which found diverticulitis in the sigmoid colon and a trace amount of fluid in the dependent pelvis. I also had a ton of blood work done, all clear except elevated bilirubin, which was diagnosed as Gilbert’s syndrome given that everything else was normal. Anyway, I’m just so frustrated that nothing has improved and scared something could have been missed. I’m hoping to have another colonoscopy as soon as possible for surveillance purposes. Has anyone had a similar situation? My rectum doesn’t feel right, but perhaps it’s the combination of the hypertrophied anal papillae (benign) with the hemorrhoids and the inflammation that caused the small diverticulitis in the sigmoid colon. Has anyone experienced similar circumstances? It feels like their is a lump or something in my anus/rectum. Would the CT scan have detected a malignant polyp in the rectum?

The CT scan (if with contrast would be better) wouldve caught any tumor of polyps. Im sure CT scans without contrast could catch those as well.

Hi, I’d recommend seeing a surgeon to check for anal fissures. Mine got missed by my gastroenterologist, multiple GPs and a colonoscopy. A colonoscopy + sigmoidoscopy+ CT scan would have caught anything that would have been serious. I wouldn’t know whether there’s a point in doing another colonoscopy this soon after as they didn’t really see anything before and it’s quite invasive. I would also maybe do an HPV check just in case since it’s the main cause for anal cancer development.
