cataract surgery for Fuchs dystrophy patient

I am having cataract surgery on my right eye on 10-28, then on my left 11-25. My cornea thickness is normal at this point so I don’t need the mini cornea transplants yet. I am currently on zanubrutinib and have been for 17 months. I have an appointment with my oncology nurse practitioner tomorrow. I am sure I will pause the zanubrutinib for a few days before and after. My lymphocytes are coming down slowly from 212000 to 21000 in July. Hopefully they will be even lower tomorrow. I have a lot of drops to use before and after surgery. My doctor is a well known cornea specialist who tells me he will do the cataract removal in a way that will protect the corneas, but there is always the possibility the corneas could get worse and I would need treatment for the Fuchs. Has anyone else had both CLL and Fuchs dystrophy and have any advise?

Unsure about if it's Fuchs dystrophy but a friend of ours has some rare corneal disease and after 2 years she's still on the wrong side of healed from her corneal transplants and has recently popped a stitch. It's a shame she's suffered for so long.

I don’t have any advice for you, but just wanted to let you know that my corneal specialist did both my cataract surgery and corneal transplant at the same time. The second eye was done a month later. That was six years ago. I was 80 at that time. My vision is now very good, and after 10 or more years not being able to drive, I had to learn how to drive all over again. I still wear glasses for astigmatism. My CLL is stable with mostly normal blood work. I have been on Imbruvica for over eight years, currently low dose. I did stop the Imbruvica for 7 days before and after surgery. I wish you well with your surgery. It sounds like you are in good hands. Maureen

I forgot to mention, my Fuch’s dystrophy was diagnosed over 30 years ago. It progressed very slowly.

I am on W&W and have Fuchs and am listed for my first cataract and mini corneal transplant at the same time in a few months. I know I will have to use ant rejection drops for some time afterward. I have lots of chest infections and wonder if the anti rejection drops will affect these. Did you have any problems like this

I do not have chest infections but have had history of sinus infections. I am now receiving ivig infusions monthly.

I have been on anti rejection drops since my surgery. I started recently to use them every other day. My doctor is okay with that. The drops have never given me any problems.

Your situation sounds a lot like mine. I have had Fuchs for many years, am also 80 and am on zanubrutinib. The reason my cornea specialist is only doing cataract removal is because my corneas have no edema yet. He does say that the surgery could make them worse, and if so we will do additional treatments. He’s hoping just the cataract removal will keep me stable for a while as a new cell therapy is in clinical trials and that is the only treatment in Japan used to treat Fuchs. Thank you for making me more comfortable about getting eye surgery while taking a btk inhibitor.

hi….i am 77 with W & W CLL…..also have Fuchs and recently had DMEK corneal transplant on left eye! Successful transplant but clear vision still lacking after 3 months! So end result is I may need glasses again eventually but perhaps after other eye has transplant.
