Mike Peters from a Welsh music band The Alarm had CLL for 29 years but was diagnosed with a Richters transformation this year.
He received treatment in the UK STELLAR trial which adds Acalabrutinib to the usual R-CHOP chemotherapy. He's had a spectacular response and his story is here
As someone who is on Acalabrutinib this is really interesting. Combining it with chemotheraphy and getting to remission is just wonderful. I wonder if remission in this case is uMRD? It will be interesting to see if the NHS will offer this combination as a 1st line treatment or only for those who have Richter's? It is wonderful alone that it dealt with his Richters as this frightens many of us.
I think offering this combination will be a long way off as the trial is recruiting only very slowly and this is putting the trial in some difficulty moving forward. It means that there may not be enough patients to be able to demonstrate the statistically significant difference and improved survival that would be needed for it to be adopted widely.
Trials in USA are using Venetoclax and that is also showing promising results.
