I was diagnosed in 2017 will CLL/ SLL. Enlarged lymph nodes biopsied. W&W since. Labs not too remarkable . The labs this week are not too surprising but the Gran% was 28. Quite a bit lower than past results. The labs were just part of a yearly checkup with my primary care Dr. I’m not scheduled to see the hematologist until November. They also detected blood in the urinalysis.

I don’t feel sick but I’m extremely fatigued. I researched Gran% but couldn’t find and answer to how low the numbers get before one should check it out. I am Trisomy 12 and was diagnosed as intermediate in 2017. If anyone has information on the significance of the Gran% numbers I would appreciate the comment.

Your granulocytes (gran) are another name for your neutrophil (neut) white blood cells. I expect your Gran%, has gone down, simply because your lymphocyte count and hence your percentage of lymphocytes has gone up - as is commonly the case with CLL over time. (Because white blood cell percentages, by definition, must total 100%, if the percentage of lymphocytes goes up, the percentages of other white blood cells must go down. This can happen even when there is a lesser increase in other white blood cells than seen in the lymphocyte count increase. That's Why, with Chronic Lymphocytic Leukaemia, CLL, White Cell percentages can be dangerously misleading. You may not be neutropenic!

Always ignore percentages and track absolute counts. The only time you need a percentage is when you not given an absolute count. Then the absolute count is percentage x the total WBC.

Thank you. My lymph count was 6.7 and the WBC 10.3. so still not too remarkable

28 * 10.3 / 100 = 2.884.

The normal range is 2.496 - 8.424

It's at the low end of normal range, you should continue to monitor it.

Hi there,

I had blood in urine which turned out to be bladder cancer. I’d suggest an appointment with a urologist to start with. Caught early it’s very treatable.
