Can a biopsy give false results without removing the previous treatment? My opinion is yes, but that's just my opinion. Lately I am seeing many people with positive biopsies and mutations.

I've read your FB post but I'm unsure precisely what you are saying the timeline was. Did they look again at the original biopsy and decide it wasn't Richter"s or biopsy after treatment for Richter's and there was no sign of it.

If Richter's is going to respond to chemo then just 3 courses would be enough to eliminate it and the rest is consolidation to be sure. It doesn't mean that it wasn't Richter's in the first place - you would need to go back to the original biopsy. In clinical trials the biopsies are often peer reviewed, so lots of experts look at each biopsy and send back their results to the trial co-ordinators.

Some cases are very difficult especially if this person previously on a BTKi?

Florid relapses on a BTKi can result in rapid enlargement of nodes, raised LDH, feeling unwell and a positive PET scan and I speak from experience here.

I know of cases where it could be a funicular lymphoma or leukemia where, when the biopsy is done without stopping the treatment, it becomes more aggressive. becomes The patient begins to be treated with chemotherapy. What I have doubts about is if he transformed because the treatment he was taking before the biopsy was not withdrawn. I think you can temporarily transform the cells from small to large if you don't remove the treatments two weeks before biopsy.
