Evusheld Reaction?

Got Evusheld today after waiting > 2 hours at St Jude for the paper work for something that was ordered 2 months ago. The shot is delivered in both gluteus maximus but my left arm hurt after. Why? That's where I got my bivalent vaccine 2 weeks early and where the Evusheld antibodies targeted. That is not a good thing as it may theoretically decrease both vaccine and Its own efficacy. No-one really knows. Anyone else have a similar story>

I haven't had Evusheld---this is news that is bad but is good to know as long as we take heed as to the timing of our Bivalent Vaccine and Evusheld to avoid what you have experienced.

I am assuming that a longer time frame between applications would be what is needed, but is that true?

I waited the recommended 2 weeks as I am leaving on a trip tomorrow, but longer would be better.

It’s interesting to me that there is no wait period between Evusheld and booster. Just wondering what happens if someone gets Evusheld followed by the booster a few days later. Seems like not a good idea.

I was told there is a 2 week minimum recommended wait time by the CDC between vaccination and Evusheld, and at least some places are *mandating* it. My Evusheld happened to be scheduled 10 days post bivalent booster, and my specialists office called the AM of the Evusheld to cancel. The nurse reviewing her patients noticed there was only a 10 day gap, and their policy is, it must be *at least* 14 days or they won't do it.

I was seeking to have Evusheld in March after becoming post COVID-19. I was told to first get my booster because having the booster within a couple of months of getting Evusheld would render the Evusheld ineffective. So, I was as Oro get the booster and after two weeks have the Evusheld injections.

I received my Evusheld in July, Bivalent (Phizer) exactly 2 months later. No reaction to either.

My timing was similar to yours. Bivalent 17 days before Evusheld. I did not have arm soreness the day of Evusheld but recall wondering why my left arm where I had the booster was sore 3 days after the Evusheld shots. I chalked it up to all the activity I was doing, gathering rock samples into a bucket. But now you have me wondering. I had planned to get a LabCorp test 4 weeks after the Evusheld to see what my antibodies are doing.

I had my first Evusheld shots months ago and will have my next the last week in December, (exactly 6 months and a few days later). I had no reaction at all. I hadn’t had a vaccine for several months before them though because I was waiting on the newest vaccine.

I had the Moderna Bivalent Covid vaccine on 9/7/22, then Evusheld on 9/26/22, a 19-day interval.  My vaccinated arm didn’t feel any different after receiving Evusheld. Maybe no more spike was being made by this time.

What I did notice earlier this year was that my anti-spike antibody titer as measured by LabCorp (Roche Elecsys Anti-SARS-CoV-2 S ) fell from >2500 on 2/2/22  to 155 U/ml  on 2/23/22. I had a Covid booster, an hour after the LabCorp blood draw in 2/2/22.   So, maybe my Covid booster-produced spike protein depleted my anti-spike antibodies, which I'd gotten from an IVIG infusion in January. ???

Very interesting. My LabCorp jumped from 3,000 about 5 months post Evusheld to 23,000 3 weeks post a full dose Moderna booster. We are all different. My bivalent was 2 months post Moderna.

5 vaccines zero antibodies. Just got over covid infection. Had my bivalent booster 5 days ago. Following day felt pretty crummy with fever. Had zero side effects from previous 5 vaccines. Was planning on Evusheld in another two weeks. Maybe I check my antibodies in a few weeks, and if they are robust, I put off Evusheld for a while?

I got the Evusheld 2 weeks to the day after the bivalent with no covid vax'd arm pain after. No AE from Evusheld either. Planning to get Lab Corp four weeks after Evusheld.

I see people talking about the labcorp antibody test, which I have done multiple times (I am a lucky one who began producing anti-bodies from the start, with them growing with each subsequent vax, but who after 2nd vax developed ITP which seems to have gone away after 4th vax.). Stanford has their own test and declares IT measures the antibodies that really fight COVID, implying the labcorp test is not as discriminating. Whatever that means, Stanford would therefore not order me Evusheld after measured in their 90-100th percentile last February. My local hema/onc got me Evusheld which I will get early next year as I just got my bivalent. I'm not taking chances of diminishing either and thanks for this post and its discussion.

curious how Stanford’s lab test differed from LabCorp’s. No body knows what level of anti-spike Abs lowers infection rate, and now with so many diverging spike types, and the new omicron spike vaccines…the whole issue becomes more muddled. That said, having self produced anti-spike Abs is certainly better than none.

Brian, replied yesterday but continue to to think about this question. One would like to think that two weeks after any vaccine all the immunogen deposited in one’s deltoid would be absorbed into circulation and thus antibodies made in reaction to them. It is curious, as you suggest, that after EVUSHELD those antibodies would create inflammation ruckus and soreness locally at the injection site.

It seems most likely that almost all immunogen that was placed in your delt has left the injection site and therefore been reacted to thereby boosting the antibody count

This question has caused me to wonder if synthetic antibodies( EVUSHELD) are counted among natural antibodies in the lab corp count?

I believe that multiple posts have commented on a significant increase in spike antibodies after Evusheld - usually 4 digit numbers.

just looked mine up, 15,500 from booster 6 weeks back and Ev 2 weeks later. Not >30K but will take it.

Hi Brian sorry to read about your situation. Let’s hope you do still benefit from both vaccines.

We still don’t get Evusheld here in the U.K. !!! much to my regret and horror, but it is still interesting to receive your advise if this should ever change 🤞.

I too will be getting Evusheld post bivalent vaccine, but the period will be greater than the 2 week minimum. I will post if I notice anything.

Interesting. I had my booster shot October 5th, will get Evusheld on October 24th. Will keep my eye on my right arm.

No Evusheld, had bivalent yesterday some 14 weeks after monovalent…weirdly, my arm that was not jabbed yesterday felt some twinges yesterday. I chalked it up to phantom pain, but is it possible something remains localized to the injection site so many weeks later? Today, I’ve forgotten about those phantom pains in the opposite arm as my injected arm has become very tender overnight.

I received the bivalent booster on 9/27 and am scheduled to have a second Evusheld this week. I will report back soon. The first time I received Evusheld was about 5 weeks after my previous booster with bo issues.

Hello Dr. Koffman,

I had Evusheld about two months ago on the same day that I got my IVIG. I had not gotten a booster since about four months before that. I did have a bad reaction though, but not in my arm. I had severe pain in my pelvis area for about 15 minutes soon after getting the shots. The nurse had me stay an extra half hour to make sure that this did not reoccur. It didn't and she told me to go to the ER if it reoccurred at home. It did and this time it lasted for a little more than an hour. It topped right before I got to the ER and never came back. They did all kinds of tests and found nothing. My oncologist said that I should not get Evusheld again.

I know this experience was totally different than yours, but just figured that I would throw it out there.

Thanks for posting this. Just got bivalent vaccine #6 yesterday and had Evusheld #2 set for a few weeks out but will move it out a bit. My oncologist did warn me to space them 14 days at least.

I had Evusheld #1 done about three months after my vaccine #4. I don’t recall much side effect and definitely not localized, just more feeling a little off kilter. Was not as bad as my COVID vaccine reactions have been.

Hi Dr. Koffman. Yes, I did have a similar situation. I received my first booster on 2/24/2022 followed by Evusheld on 3/11/2022, (150 as opposed to the 300 strength, as that was the recommended dose at the time) Both injections were in the butt, yet my arm where I had received the booster was sore approximately 12 hours. I mentioned it to Dr. Matthew Davids (Dana Farber) who seemed a bit surprised.

So I had my Evusheld and Prevnar 20 on the 25th, and my bivalent Covid booster was on Sep 24. A solid month afterward. My left arm was sore for only 2 days. I'll mention the RN did the Evusheld too fast IMO, I could feel something running down my butt, and at first I thought I was bleeding. She said "oh no, it does that, runs out a bit" I tried to be calm as I pointed out "but that means I am not getting all the drug in me, perhaps you should do a slower IM push and wait a few seconds before withdrawing the needle". Duh. Like, she KNEW that was occurring in her patients, yet she hadn't thought to ask her charge nurse (she was very young) or comment to someone. It literally did not occur to her that drug running out of the injection point was something to be avoided. She knows now, but that doesn't help me or her earlier patients. She even commented "oh yes, I usually do a Z track to avoid this, but I haven't been with the Evusheld".😳
