Reduction of Calquence to 100 mg per day

Please any chance to let me know, if anyone has reduced the above mentioned from 200 mg per day to 100 mg per day, because of bone and muscle pain?

If so, any better results in reduction of overall pain?

Very much appreciate any answer.

Hi, I reduced to 100mg per day for three days and had a total break for another three days as I was having ongoing stomach pain. Happy to say it gave my body a chance to ‘reset’ and having restarted the full dose on day seven the pain has only occasionally returned before disappearing after a short time with no need to change the dose. Hopefully, you’ll find the reduced dose helps and you can return to the full dose if possible.

Acalabrutinib is twice a day for a reason. Choosing once a day may compromise the effectiveness of your treatment due to the very short half life of the drug, i.e. the short time that a single dose works in your body. Do not reduce dose without consultation with your specialist.

And BTW, you might get more input if you lock your post.

My husband went down to one a day due to severe fatigue. He did that after 4 Rituxan infusions. He is currently in a form of remission even with only taking one a day.

Hi Traudel I am on the Assure Trial for 12 mths now and over 10 mths on half a dose eg 100 mg per day . This was because of bleeding issues mainly . That stopped when on half dose . No other serious AE's . I had to stop the Acala 3 times for other treatment issues for a total of six weeks ( spread over time) under instruction from the Trial leader . My results have been amazingly good. WBC from a high of 165 down to 16.95 , Platelets 70 to 135 , Lymphocites 155 to 12 etc. I did have a little fatigue for about 2 or 3 mths , found my Iron levels were just on the lower side of normal took 325 mgs ferrous sulfate tablet once a day . Would you believe the very next day no more fatigue and has been like that ever since.

A World class Specialist at Peter Mac Cancer Centre in Melbourne Victoria whom I still consult with as well as the Trial Dr in my own State , stated AstraZenca tend to over dose anyway and a half dose is fine. He was also was unaware of any resistance issues by being on a half dose .

Again I very happy with the results a great drug in my case .

This is my fifth day on a single dose and need to report, I'm so much better.

My rolling muscle und bone pain have subsided to a minimum.

Hope only my bloodwork will be positive.

My husband also had his dosage reduced by his oncologist late last year. He was in treatment with Gazyva for a couple of months at that time, but had already started Calquence a couple months before that. The morning dose of Calquence would result in such body aches for him, it made much of the day a total loss for him.

With his excellent response to Gazyva, the Calquence dosage was reduced. He takes just his nighttime dose, has about an hour of a heat flush and body/joint aches, but then can take a sleeping aide and sleep through the bulk of it. There's still some residual effect in the morning, but usually by 10/11 am he's ready to go. (With that said, he has other pain causing health issues such as degenerative disease in his spine. Sometimes it's hard to say what is hurting exactly and why - but we definitely saw an improvement with the elimination of the morning dose.)

He finished his infusions in January and has maintained clinical remission so far with the one dose. He'd like to get off of it completely, but as he's only 54 years old, has had CLL 7+ years, and had developed worse prognostics just last year so the oncologist wants him to stay on it to push remission as far as possible.

Hope you get relief! Great drug, but some cost for sure.

I have been taking 100mg for almost two years and it works no problem. I have trouble taking pills so I open the capsule and put it in my yogurt. I talked to manufacture and they said the reason they suggest taking the capsule is because they did not tested any other way. Even with the 100mg last three months I started to have some same symptoms as I had on Imbruvica. Joint pain and rash. I tried to reduce dose to 50mg for the last three months and my WBC went from normal to slightly above normal range. I will try 100mg one day and 50 next day and talked to my hematologist about the issue next time I see him.

Hello Pragnar, Thank you so much for your information. It's so very interesting. My overall pain has been so much reduced, for now I'm very happy to have reduced Calquence. Hope my next blood work will be not showing surprises.
