Workout QR scan

Does anyone recommend an app (no smartwatch/garmin equip) that can scan QR codes on gym equipment to save workout details? This is actually for my mum who’s using adapted equipment in a wellness programme and I’d love something that her iPhone could scan and show progress but can’t see any apps jumping out on the store! Tips appreciated :)
