I suggest you and your friend read post linked below

The short version is we are all different, who we are, age sex, medical conditions, where we are going and for how long etc etc so it pays to do your own research since insurance providers assess risk on an individual basis. So just because someone on here has had a good deal with Avanti or Staysure or Allclear or any other provider doesn't necessarily mean everyone will. Therefore my suggestion is to first consult a website for travel insurance for pre existing conditions and take it from there or contact a local broker .
As for cruises they are tricky due to the increased risk of having to take people off the ship should they become ill and in that case it might worth contacting the cruise company to see if they offer insurance
as Lowerfield says, plus the website Moneyhelper.
Give Avanti a try
I got travel insurance a few weeks ago with Allianz through Lloyds bank.
They were brilliant and very understanding.
Give them a try. Good luck
