I now have jsut discovered that in the comments part of my report it says cabg x4 which mean quadrouple bypass this is a complete nonsense unless you can do a quadrouple bypass when inserting a pacemaker in my shoulder. the only other surgery is a gall bladder removal

Mistakes happen! Just report it to the cardiology team and also to your GP and don't get too stressed by it. In the grand scheme of things, its minor.

As Steve says do not get stressed out over it - these things happen. When I had to have an infected tooth out in April I got to the oral surgeon to find he had been given told the wrong tooth! Funniest was when the GP told me that I definitely needed to lose weight - it turned the nurse had lost 10cm off my height!
