Unsure why you think your brother May have Level 4 COPD?
Its down to a professional diagnoses, with tests and scan to confirm the diagnosis.
I wonder if you are jumping the gun a bit and worrying unnecessarily if the diagnosis is yet to be confirmed.
The best person to advise your brother is his consultant, or go to the Lung Foundation in the country you reside for more information and contact with their helpline.
UK BLF information and helpline is here: blf.org.uk/
Generally some who smoke expire sooner than those who do not and those who look after themselves, with regular exercise diet, avoiding contact with those infected with cold or flu virus (not to mention covid) are more likely to live longer with a better quality of life.
A pulmonary rehabilitation course provides information and education on how to manage symptoms and slow the process of the disease. Pulmonary Rehabilitation in UK at the time of the pandemic may be available as an on line course through medical professional referral (GP or hospital Consultant)
Best wishes to you and your brother
