Someone just shared a link with me to a post on Reddit about a 'Chronic Illness Resume' that's meant to help with medical appointments.
It's essentially a one-page document that includes a list of goals, treatments, questions, etc. Pretty much all the stuff you might forget to bring up at an appointment with the Doctor.
I've had trouble communicating with doctors in the past, so I can see how it might be useful. In fact, I once got laughed at by my GP because "men don't get eating disorders" but that's a whole other story.
