I struggle with this too. I just try to be realistic about what really needs to be done and make a list. Set a timer for 20 minutes and start one thing. When the time is up I can choose to keep working on it or switch to another task, or take a walk or drink some water, etc. If I don't start somewhere I'll just keep worrying about getting things done and never get anything done...so the timer helps me get focused and I can do anything for 20 minutes!
Simplify, always simplify. And making lists so that my mind is clear. When I know my to-do list is on paper and I don't have to keep trying to remember things I'm free to focus on the moment more.
