want to know if restarting abiratoerone is helpful

I was diagnosed with T3 prostate cancer that is regional. that is spread to seminal vesicules and

bladder neck invasion is suspected. Gleason score 5 plus 4

Started with Orgovyx and then added Abiraterone 1000 mg. Prostate cancer is reduced in size by 30%.

Had 23.2 gy proton radiation .Finished it on March 13th. Stopped Abiraterone on Feb 4th due to Liver enzymes shooting up.

Going for SBRT on March 26th. MY PSA is less than 0.03 and my Testosterone is less than 4ng. on March 14th.

wondering if I can avoid taking abiraterone as the liver enzymes go up even if I take 500mg.

How to decide? The oncologist suggests starting abiraterone at reduced levels of 500 mg and then stopping again if the liver enzymes shoot up. Already, liver enzymes shot up twice even with the reduced dose and I was advised to stop Abiraterone .

Anyone has been in this situation? Another doctor suggested simply Orgyvox is enough if continued for two years .

Any advice, please?

Did you take prednison in combination with Abiraterone? I agree with your doctor, after radiation Orgovyx will be sufficient.

I started with 7 months of 1000mg of Abi and my liver numbers skyrocketed. Went off for 7 weeks. Restarted at 500mg, all ok. Upped to 750mg and stayed there, all ok. Per instructions your liver numbers (ALT & AST) can go to about 5 times normal. Best of luck.

Thank you. They went more than 6 times . That is why doctor stopped it. Now the liver enzymes are normal and I am wondering if I need to restart or leave it since I am undergoing regular as well as SBRT radiation.

hi

I took Abi 1000 for 3 months or so and ALT slowly crept up to around 180 so I stopped

This was at start of EBRT Since then I have tried half dose etc and it was going up a lot but also taking quite a while to come down

Now I have started 250mg every other day and it shot to 250 in a week but dropped to 100 in a week and down to 40. So trying to increase dose intermittently as opposed to letting ALT go sky high

Depending on the studies you read, around 6- 13% of men are taken off because of quite severe liver toxicity

with a PSA less than .03, how were your Mets identified/located enough to do the precise radiation?

I did the radiation at MD Anderson they did a MRI and CT scan simulation and did the radiation and I am going to do SBRT in a week for 3 sessions. Thank you indeed

I started 1000 Abbie and Pred back in August 2024. Alt/AST 5-6 times higher in Dec. Stopped for 6 weeks went back on 1000 and same thing happened. I am off Abbie again and just doing 3-month Eligard shots and PSA is still less than .04 and MO seems to think we stay with just that until PSA rises. There are studies provided by TA that show that people on ADT and Abbie or other drug live longer than just ADT alone. At my next visit I will discuss going on reduced Abbie 500 or maybe switch to xtandi. I believe that doublet treatment is better than ADT alone and hopefully will find a drug that I can tolerate.

Thank you indeed I will discuss with my MO tomorrow about this and let you know. Yes you are right studies show both Orgyvox and Abi make OS longer. I am looking for less ways to prevent recurrence by sone other means than Abi. Heal Navigator is a good source for that . SBRT is supposed to reduce the duration I have to be on ADT. A lot depends on luck I think .
