I am 85 and stage 4 with bone mets. I am presently taking Nubeqa and ADT injections every 6 months. After over a year and a half, I am responding well to treatment, side effects are minimal and manageable and my QOL is good.
All that said, the idea is very appealing to me to have an orchiectomy instead of continuing Lupron. I would appreciate your insight about this… good idea or not, and why.

It’s up to you. Personally I can see it as a convenience to just not need the shot. I don’t do an orchidectomy because I get the injection when I am at clinic for my visit with oncologist, so it’s not inconvenient for me. The 3 month Eligard injection is fairly painless, occasionally it stings for 2-3 minutes, but no big deal.
i’m also stage four with Mets to pelvic bones. Diagnosed at age 61. after 3 years of doing the shots I knew I would be on it for life so I did the orchiectomy, no regrets.
I did it after 7 years of ADT and getting rid of the side effects was the primary reason.
If your QOL is good (check with your partner, it might be a "forest for the trees" kind of thing) and the shots are not a problem, then you do need to consider that there is always a chance of infection after any surgical procedure. Very small chance.
I feel that my mind is clearer and my spirits better, so overall increase in QOL.
