Has anyone tried Bipolar androgen Therapy ( BAT) that involved administering testosterone for re-sensitization? Maybe you are/were part of the TRANSFORMER or the COMBAT clinical trials that tested this strategy? I Want to hear lived experiences so we can make informed decisions.
Dad's (70, Gleason 9, mCRPC) disease is progressing despite 2 rounds of Pluvicto so doctor has recommended stopping pluvicto. Doctor has suggested BAT and we were meeting soon to discuss more.
Based on my reading the clinical trial was mainly restricted to individuals with CRPC who were non symptomatic. Dad has lesions in lungs and nodes around it, and one rib. Dad is frail and experiencing breathing issues and I would not want to put him in any more pain.

There are better clinical trial options. Although I'm hopeful for BAT overall, it's best used when there are no major symptoms as you saw.
There are Cu67 and Ac225 radioligand trials underway for patients who have failed Pluvicto. Some of them use different targets or ligands so might work when Pluvicto doesn't. That would be the first place I'd look for a next move.

Second, you are correct, most of the men who have been treated were asymptomatic CRPC men with or without mets.

Before going with your doctor's advice I'd encourage you to consult with Dr. Samuel Denmeade, Johns Hopkins. He's the expert on BAT.
Last, there is a way to test out your father's response to high testosterone without going through an entire month of possible pain and progression. PM me if you'd like details. The method I am referring to has been tried by a few people.
