I have been asked if I would like to participate in a trial. I’m thinking docetaxel and enzalutimide have been studied to death. I have some ideas what i want my treatment to be now that I am mCRPC. Thoughts? This is from the trial literature

After abiraterone, the standard of care for your disease is either chemotherapy with docetaxel, or another hormonal therapy such as enzalutamide. Enzalutamide is a well-tolerated therapy but only a few patients will benefit from it. More patients will respond to chemotherapy, but it has more side effects.

Plasma circulating tumour DNA (ctDNA) is tumour DNA released by your cancer into the blood stream.Previous research has shown that patients do poorly if they have a high level of ctDNA before starting anew treatment. In our experience, enzalutamide is less effective if patients were previously treated with abiraterone AND they have high level of ctDNA. On the contrary, patients receiving docetaxel after abiraterone have similar outcomes regardless of their starting ctDNA level.

Purpose

The purpose of this study is to find out if participants would benefit more from a strategy where treatment decision is based on ctDNA level rather than clinician’s choice. In this Phase II study, participants in Arm A will receive docetaxel if their ctDNA level is 2% or greater, and enzalutamide if their ctDNA level is less than 2%. Participants in Arm B will receive either enzalutamide or docetaxel based on your cancer doctor’s assessment.

enzalutamide works longer when combined with docetaxel,, even if you are enzalutamide-resistant:

Exactly. i did mention the PRESIDE trial. I don’t think this trial will benefit me. Seems like they should already know the results.

I meant the PROTRACT trial won’t benefit me. I am up for the PRESIDE trial protocol as It would have some benefits. I am all for being a guinea pig but at something that may benefit me as well as research.

I think it is the PROTRACT study NCT04015622 clinicaltrials.gov/study/NC... Currently you have the choice between Enzalutamide and Docetaxel. Enzalutamide will usually work for two to three months and then it is followed by Docetaxel anyway. The study tries to determine if Enzalutamide works longer than two or three months if the ctDNA is less than 2%.

Taking part in the study will not make a great difference for you. You may just help Dr. Kim Chi and take part in it. It you get into the control arm you can do whatever you want.

Yes it is the PROTRACT trial. I want to talk to the MO as I don’t want this interfering with my chosen treatment.

Either enza with chemo at the same time or participate in an Actinium trial. there is one going on now at BC Cancer. I don’t know if I meet the eligibility requirements.

I would try for the Actinium trial if possible. Is there a link to it? Hopefully you don’t have to have already failed chemotherapy.

NCT06217822 is the trial number. This is all I can do. I am a techno peasant and can’t post a link. You can probably google it. There is one group (B) where you don’t have to do chemo first.

Actinum for sure. They have used it at my hospital for 6 month now and the results are promising!

Once you have done your research on the benefits and potential side effects of a clinical trial, keep in mind that once you sign on, you probably will get VIP follow up with the medical team conducting the trial...I have been on phase 3 trial AZD5305 (ABI + PARPi + 5mg. pred) for 6 months and have received excellent follow up from the Odette Centre at Sunnybrook hospital in Toronto...Recent numbers look and feel like it's working...Nothing to loose gsun...

I am exploring the ivermectin/fenbez treatment. Started Fenbendazole today. Been on no sugar diet for 4 weeks to starve the cancer out.

Hundreds of cancer patients in all stages completely cured including my Army buddy’s mom and my sisters friend. Good luck but my father in law did a clinical trial after living 10 years with it and was dead 3 months later.

PCa is quite different from other cancers, I call it the Hydra. If you are shunning SOC in lieu of fenben et al, it probably will be a painful sayonara for you. The Hydra cares not in the slightest.

Your army buddies mom had prostate cancer? Hmm…

If a no sugar diet starved the cancer, we wouldn’t have to get treatment. However, when I gave up sugar for a month, my PSA did not go up like it did when I ate sugar. Going to try it again

I am not being flippant but how exactly can you give up sugar? It is in virtually every food and drink we consume. I think giving up refined sugar makes sense. I also believe high fiber carbs can ameliorate some of the negative effects of sugar. I am addicted to something sweet with my coffee but have been trying to cut other places.

Yes, refined sugar. Carbs are a problem too. I ate more eggs, tuna fish without bread or with a single slice. Never went overboard though. Didn’t cut out fruit.

My friends mom had blood cancer. My sisters friend had stage 4 bone and liver cancer. Neither did any traditional methods. Both got healed by this diet plus ivermectin and a few other lifestyle changes.

This is what I am doing and by no means medical advice.

No bread, no coffee, no milk, no gluten, etc. absolutely no sweets or fast food whatsoever like cookies, chocolate, etc; no potatoes pasta either. I keep carbs under 20 most days. I drank 4 cups of coffee a day with light cream. But I cut it out completely.

Sugar feeds cancer. Keep good fruits(blueberries /strawberries)to 1/2 cup max day total because they have sugar but need the antioxidants. I drink green decaffeinated tea now(I hate green tea but doing it with the flavored ones-mint and lemon) , no orange juice. 1/2 glass of sugar free cranberry/pomegranate juice; beans, walnuts, almonds are ok in small quantities. Lots of salads. No croutons and only balsamic /olive oil dressing. I cut out red meat 90 percent. Replaced with Salmon 3 X week, chicken and ground turkey vs ground beef. I’ve been on it for 32 days. Lost 12 lbs. The pains in my lower abdomen area and balls stopped today, and I held my piss for over 2 hours- first time in 6 months. I was pissing every 20 mins and had the constant pain everywhere in my body . I still have slight lower back and neck pain but this also has been reduced. I Exercise min 45 mins day(cardio) This has been tough , but gotta push through and work up to it if it’s hard. Cancer hates oxygen. I added high doses of 10,000 vitamin D, (until my blood reads 80 for D then will cut to 2000); regular dose of zinc, 1000 Vitamin C, tumurac and raw veggies plain every day(broccoli ,tomatoes and red peppers). I Added 444 mg of fenbendazole today.

Today is the best day I’ve felt in a while. My energy levels also increased. I see the cancer clinic tomorrow morning for their next recommendations. I will get my psa test in 3 weeks. Last score in Nov was 13.5 total and 6 Free with 10 Gleason score from biopsy 2 weeks ago. If my total and free start reversing significantly, then I will continue. I am not doing any drugs, radiation or chemo now. Going with a radical lifestyle change with diet, parasite killers, and my faith in Jesus Christ until next psa test and Pscan. If I look like a fool then , so be it. But for now, gonna keep pushing this.

Let me know how the ivermectin works for you. Pm me. I’m looking at similar on my 3/ 6 doxecetal round W low PSA. I had done the keto diet for three months not knowing my PSA had increased back four months ago before I started back on treatment. Would like to know how yours works

Definitely will. I am on keto now and in process of 3 day water fast. . Finally found a good source of ivermectin with good prices but won’t get delivered for 2 weeks. I will be on 90 mg of ivermectin daily(for 200 lbs) with the 444 daily fenbendazole, and continue other vitamins and keto/ fasting.

And of course- no booze of any kind. lol. I used to drink a lot. Cut it all out. I can’t believe 32 days already. But cancer will wake a person up to do radical changes.
