sorry to be a thorn .. always read sometimes comment ..

need to update bio sorry.. brachytherapy seeds, Oct 2023, ADT prior from June 2023 … psa 18, jumped from 12 a year before. MRI biopsy one lobe lesion nerf left capsule edge supposedly didn’t breach with a few cores 3+4, 4+3, 3+3 on one side only. ..no known spread via bone and pet scan.. unfortunately no pmsa pet scans here so have to wait later. Things seems to went well last linac treatment in Jan 2024. Stopped adt March . So my question is around psa scores and reoccurrence.. had .001 April .003 in July , and .23 in Oct so a big spike but also when my testosterone started to recover .. not sure if this is a bounce but seems too early. I’m aware of the .2 or less favorable study 2-3 years out for Ndir .. I guess i crossed it quickly not sure what to make of this .. it’s 2 consecutive upticks right? Will have to wait for next PSMA in Jan .. but seems a bit early for a rise ..maybe a spike ..hopefully a drop. I guess I wonder as if you still have a prostate with some viable cells and produce testosterone.. .03, .01 etc would go up after treatment and ADT weans off. I realize 2.0 psa is failure so just wondering if this is common and a potential issue. If an issue and rises ..plan to go somewhere that offers PSMA pet scans to see if anything spread and treat accordingly. Originally my psa jumped from 12 to 18 prior to treatment. Anyway any input appreciated.. I guess I’ll know more in Jan. Best to all.

glad to hear you are still here with us fighters… a little about me… 2017 Jan my PSA was 8, jumped to 12 in July, then in dec to 54… my GP sent me to a urologist who after having a field day probing (he has really big hands) found nothing unusual… so off I went to biopsy… afterwards, I complained to the department head that I just lost my virginity (the biopsy tool they used was humongous) hahaha… sorry for the humor… out if 12 shots they found 10 with 100% stage 4 malignant something or the other…

Went and had the bone scan… cancer only in the prostate… no spread… so off to the oncologist… a young fellow… who tell me my options… removal or treatments with radiation… he was really great even to the point of making a joke when I asked him this question… doc, after the radiation treatment, then the removal of testosterone by injection every 6 months (clinical castration) what will I do with my 40 year old wife??? (I was 64 at the time) my youthful Doc answered “ “when she feels like it… give her 🌺 flowers “😂😂😂😇🙏

From 2018 until March of 2023 I literally had no balls… I went into menopause… sweating hot flashes… mood swings…

What kept me go was my faith, my wife and 3 kids (who were all under 12 yrs old) my mindset and work… through it all I never stopped working as an automotive engineering consultant…

So March of 23 my youthful oncologist told me not to die from anything else & handed me to my GP for annual inspections 😩😇 my PSA throughout the 5 yrs have stayed below .003

My only drawback is my once magnificent manhood only stands at 1/4 mast (military flag raising ceremony) Half mast full mast ect… I’ve never tried pumping… my wife helps me a-lot… if not I use other methods to please her…. Using kegel exercises helps to strengthen my lowered flag pole… I’ve started taking a supplement that helps a great deal… but I had my GP check it out just incase it may raise my testosterone too high and the beast May return…

Oh well so long for now… my 3 little ones are 51 boy, 13 girl and 12 boy… great kids … love you all … God Bless

sounds like you did well .. humongous was an unfortunate experience but glad it’s good .. with kids yes a bigger picture .. depending on your age 60s or so try a pump or two what do you have to lose.

oh by the way… I eat 2 apples with the seed core or stone… don’t believe what big pharmaceutical is saying about how dangerous the apple seed stone is…you will have to eat the seed stone of approximately 600 apples to die from apple seed stone…

BTW Johnny Appleseed did not die from eating seed core/stone it was his drinking 4 gallons of apple cider vinegar daily that got him.....

I am disappointed that your care provider has not discussed with you the PSA bounce that occurs after brachy. There are many papers that discuss the range and distribution of PSA after treatment and project outcomes beyond initial therapy. I would ask your radiotherapist to explain what is happening. I think the initial post treatment score of 0.003 is in the unmeasurable range. Increasing to 0.2 and holding is still a very, very low PSA score. For peace of mind, seek assurance from a knowledgable and compassionate doctor.

Thanks for the reply. I’m living in Japan where proper communication is difficult and the doctor culture is also different. I’ve made a point to research prior to the follow ups and relay them to my fluent speaking wife but that too can be a challenge. I guess my NDIR is 2 and that’s the threshold for reoccurrence. Having been on ADT for 9 months and off now for 7.. would expect a bounce especially since I have a prostate. Most of what I’ve read though is that the bounce happens a year or more out. I’m wondering what others might know or have experienced close to a year out regarding expected PSA scores. When I read about doubling scores then my .001, .003, to .23 is an 8 fold increase. However still being impacted by the hormone time duration testosterone didn’t return or perhaps started to? I anticipate the next reading to tell more. At some point post PSA .5 <1 will get a PSMA pet scan outside of the country.. not readily available here yet. Best to all.
