Been on Zytiga over a year, taken with Xgeva and Lupron. During the two pill test period of Zytiga, no food - 2hrs and + 1hr, my T and PSA became undetectable. Convinced the powers to continue with 2 pills to possibly reduce side effects and prolong effectiveness. T and PSA undetectable after a year. Looking at undetectable variants of PC, is that a wrong thing to do?

Yes, it is definitely a wrong thing to do if your goal is to "prolong effectiveness." You are doing the opposite. Full dose abiraterone optimally delays time to castration resistance and increases survival.
This is interesting, and while I trust your incredible knowledge, can you explain why this is true? I have similar thoughts as I wolf down my 4 (!) 250mg Abiraterone horse pills.
It has been proven to be true in multiple trials. We have lots of evidence now that maximally and systemically reducing the cancer load has a much greater impact on long-term survival. I think of it like taking not enough antibiotic for a bacterial infection - it encourages survival of resistant bacteria. The dose has been tested to be optimal. As they say on the internet - FAFO.
You'll get a very divided response on this forum to taking 2 AA pills daily instead of 4.
If you monitor PSA very regularly and also have PET scans at least every six months, many are probably doing some version of that.
But others will say using 4 pills daily (no food) was what was used in the original clinical trials (in many men with many types of health conditions and aggressiveness of cancer), so that's what you should do.
Some take 1 pill with food and seem to do well, but that idea can also draw heavy fire on the forum here.
More is NOT better. Docs just don't care if their patient gets castrate resistant faster. They only care to minimise the time they have to spend with him.
Wow. Pretty bleak view of human existence. I was a physician and I cared. Most of the physicians I knew cared although I knew some that did not. Physicians are people and like all people there are people with compassion and those that seem to lack compassion. In this case the randomized placebo controlled trials show that hitting it hard right from the start prolongs life.
Procrustean logic, if you know the term, else, Google it. All men originate from a single "factory" that has not changed the "manufacturing" process since the inception of time. All respond to any drug in exactly the same way. Patient individualized medicine calls for knowledge, care and time not available. Magic numbers and "one size fits all" approaches may be optimal for the medical professionals but not for the non-billioner patient. Fast food business tactics presented as scientifically backed. Sloppy empiricism at best.
