Xtandi did not work.

Hi all, I'm new to this board but would love some feedback. My father, 80, has been dealing with prostate cancer for a few years. He had initially had radiation plus Lupron injections.

His PSA started rising again a few years ago and he wound up on Lupron regularly. His PSA has been slowly rising over the past year and it shot up to a PSA of 12.56 back in January. His doctor added Xtandi to his regimen and per his most recent blood tests -yesterday- his PSA is at 19.24!

Per his scan earlier this year there is no metastasis . What is the next likely treatment option? Another drug like Xtandi? Radiation? Chemo?

It is my personal opinion. It is not a recommendation. I am trying to address the next likely treatment option.

- Clinical trials that focus on ADC or AMG 509. It may add more effective results for Xtandi

- Pluvicto: It is a target therapy that can target the cancer that spreads in BONE

- Provenge: PSA is less than 25, and may be proven to kill the cancer cells as well.

Another option is to get a PSMA PET Scan report and see if anything can be radiated in the spots. Then, continue with standing + clinical trials that focus on ADC.

Thank you for your response! So far, per his last scan , it has not spread to the bone. Is something like Pluvicto able to be used as treatment for a non-metastatic case ?
