Castrate levels of Testosterone

My husband's testosterone level is 3nmol/L (86.5 ng/100ml). In the past year, his levels have ranged between 2-8 nmol/L. He's on quarterly Zoladex. I understand that castrate levels are ideally < 20 ng/100ml.

If I understand correctly, his testosterone should be much lower. Can anyone please shed some light on this for me? Should he consider and orchiectomy? He is castrate resistant with very aggressive PCa (PSA presently 1.4 with PSADT about 2 weeks, but presently declining with Docetaxel Chemo)

Thank you!

My apologies! My error! Testosterone is 0.3 not 3 nmol/L which moves the decimal by one place to 8.6 ng/100ml. A much better number!!
