My father was scheduled for a simple robotic prostatectomy which turned in a partial cystectomy .. The surgeon found a well oval encapsulated mass in the bladder in the size of 14 cm which he removed it . He also did a simple prostatectomy , his prostate was enlarged but not as big as the doctor thought and no signs of cancer in it… Also he mentioned that it doesn’t look like bladder cancer and he thinks it might prostate cancer or a muscle cancer … I am terrified… Has anyone been through this ??

A. Tumor for frozen section, biopsy:
− Malignant spindle cell neoplasm, see comment. B. Bladder tumor, partial cystectomy:
− Malignant spindle cell neoplasm, see comment. − 12.9 cm in greatest dimension.
− Margins positive for neoplasm.
− Benign urothelium.
− Separate fragment of fibromuscular tissue consistent with detrusor.
Comment: Immunohistochemical stains demonstrate the tumor cells to be positive for CD34 and negative for calretinin, S100, desmin, smooth muscle actin, CK5, GATA3, MART1, pankeratin, and p63. This immunoprofile is nonspecific. Given the history of high-grade prostatic adenocarcinoma, this could represent a sarcomatoid component of prostatic carcinosarcoma, though no epithelial differentiation is identified within the sampled tissue. The differential also includes a primary sarcoma as well as other malignant spindle cell lesions such as a malignant solitary fibrous tumor. This case will be submitted for expert.
