Hello Everyone,

I am reading this forum every day and getting very valuable information. Thank you so much. My hats off to all of you for sharing your experience and knowledge over here and providing much-needed knowledge and compassion to each other.

My Case:

First diagnosed @ age 71 with PCA in August 2019 (By Physician) Biopsy done in October 2019.

Biopsy results: 5 of 12 GS 10, 2 of 12 GS 9, and 5 of 12 Benign.

PSA 3.5.

Scans Results: Only one Met Increased uptake in S1.

Genetic test: negative, MSI High

My case was reviewed by three MOs and it was concluded that although I have a high Gleason score, based on CHAARTED and LATTITUDE I have low -volume low-risk disease (no visceral involvement, 1 site of bony disease). So it was recommended to follow the STAMPEDE trial.

Treatments:

1)Lupron shot every 3 months effective December 2019.

2) Abiraterone 250 mg with a low-fat breakfast

3)Radiation to Pelvic 4 weeks

PSA Results:

Entire 2020 and 2021 <0.03

5/10 0.21

PSMA scans done , Found one small lesion in front first rib

6/10 0.22

Radiation treatment to front first rib

8/10/22 < 0.03

11/16/22 0.14

12/29/22 0.23

PSMA Scans, Pet scans, CT Scans Findings: A new very small found in back of the first rib but report says "it may be post effect of treatment done June 2022."

MO said "Zytiga is not working and start Keytruda since I am MSI high." However, Radiologist differed and suggested to try radiation treatment to that one. I agreed to that.

Life Style:

Exercise: Walking 5 miles/day + workout with a trainer

Diet: Plant-based + Fish every other day

Side effects due to ADT: Very little. No Fatigue, no hot flashes, no weight gain.

Questions:

MO says "Zytiga is not working and start Keytruda for 2 years" and in his opinion it will put cancer in long remission and he may take me off ADT. But I like to postpone that treatment as long as I can due to life changing side effects in 10% of the patients.

So far my quality of life is not affected and My PSA numbers are not that high. So up to what numbers shall I wait to start other treatments?

Shall I try vaccine first before starting Keytruda?

Keytruda has had almost miraculous effectiveness in men who are MSI-high.

Thanks Tall.Allen. I know eventually I have to go for that. Just worrying about falling into that 10% category. But 90% odd has better outcome.

It's a very rare genetic mutation.

MSH2 defect here. Keytruda killed my cancer.
