I am looking for some relatable experiences. My husband was diagnosed with prostate cancer in February. He is 42 years old. His Gleason score was 9, PSA 17.8. He had surgery in March. He had multiple positive margins and one positive lymph node. He started hormone therapy 1 month prior to surgery. We just saw the radiation oncologist and will be starting 6.5 weeks of radiation and 2 years of hormone therapy. PMSA Scan to be done in 2 weeks.
Any similar situations and positive outcomes? Has anyone over come ED after 2 years of hormone therapy?

I was diagnosed at 56 (see Bio for details). I am just 2 years out from my ADT and almost 2,years out from HDR Brachytherapy and EBRT. We are now in the watch and monitor phase when/ if Testosterone recovers if I will have a durable remission. I have With a decent QOL. I would suggest asking your husbands MO or urologist to start a low dose viagra. This has helped me to avoid scar tissue and maintain blood flow to my penis. I still need Tri- Mix injections for a consistent erections for intercourse. My apologies I am not out far enough to provide insight on ED recovery post ADT and radiation. I am happy to provide any additional insights to my treatment to date and managing ED while on ADT.
Many of us take 5mg/day of Cialis while on ADT for the same purpose. That's theoretically enough to maintain penile health, and he can always take more for "date night." Good luck to both of you -- my condition at diagnosis was similar, but I was 69 at the time (a year ago). I had proton beam therapy, and am now 11 months into my planned 2 years of ADT (Eligard and abiraterone+prednisone.
40 mg daily, more on ‘date night’, of Sildenafil (Viagra) has the same effect for me. I was diagnosed 5 1/2 years ago. I’ve been on ADT intermittently since then and have a round of 20 sessions of RT. Details in my profile.
Hormone therapy will lower his libido, but may not affect his erectile function. Daily low-dose ED meds may be protective. If he can overcome his indifference to sex, it may preserve his size and function if he uses it more.
I have had 21 months ADT treatment. It ended any hope of my recovering Erectile function. I wished I had moved earlier on my prosthesis Penile implant (IPP). The radiation and ADT had a big impact. Here is my story and what I would have done differently.
He also needs to be aware of the chance of losing his testosterone after long term treatment. It can happen but with his age should not mine at 68 has finally started to come back after 6 months of stopping ADT treatment

I was 46 with Gleason 9, multiple positive margins and one lymph, periprostatic, infected. He is very young at 42 obviously. Tough blow, tell him hang in there, its not the death sentence that it was years ago.
I did viagra, penile injections, the dreaded pump for 2 years, and that went south as the base of my penis began to break down.
I opted for the penile implant. It was a game changer, although not a fun surgery. Ive had it for 6 years and it works great. This was just the route I went and I know its kind of a last resort type thing.
