I had the same experience in 2021. Docetaxel brought my PSA down quite a bit. Had to stop Docetaxel after round 4 because of the side effects. Doc changed to Cabazitaxel but unfortunately PSA kept going up. I would give it another try, my Doc said sometimes it takes a few sessions. Hope it works better for you then for me.
If it were me, I'd go back to Docetaxel if you can for next round since it worked once and it's been over 2 years ago so should be able to rechallenge at this time.
You could give Cabazitaxel another round, but I doubt it's working for you and it would be wasting precious time. My experience was similar to yours. I got a little out of it, but not much.
My doctor suggested to me rechallenging Docetaxel. I said, "Yeah, but isn't Cabazitaxel a better chemotherapy. " He basically said that whatever works is better and you just have to try and see.
mu husband also had great success with Docetaxel, and same as you with Cabazitaxel, initially went up then came back down to where we started and didn’t change. We were told you couldn’t have Docetaxel again but 8 years later his oncologist says it’s worth a try. He starts Lutetium 177 next week in London then Docetaxel if no success with LU177
I thought I had read that Chemo (Docetaxel/Cabaz) kills PCa, and that dead and dying PCa cells give off lots of PSA. I was told to wait 3 months after Radiation to my Prostate for that exact reason. Said it would cause a big false positive but later come down which it has.
From everything I have read, LU-177/LUJ597 are very effective at lowering the amount of PCa and PSA levels but are much easier on the body with less side effects (especially when done early) than Chemo therapy. Think of LU as liquid radiation, and Docetaxel/Cabazetaxel as liquid chemicals.
Personally, I lean going with LU before Chemo. It is always a changing field of what first, but being early seems almost universal. Mike
